Probably because there is nothing out of the city to see.  If there was a way out of the city, the game would have to be programmed with more scenery, props, etc. and there is only so much you can cram into one game.

I guess if you had too much memory on one server when you're making a game it would be a disaster if your main server crashed in the middle of it. But the only time I can think of going out of a city is in GTA SA when you fly to liberty city, but you can't go back.

That's exactly it.  Not just for the computer that creates it, but also imagine the load your console would have to take trying to remember all that detail.
